Chronic illness project

    Do you help people manage their chronic conditions? Do you work in Perth and surrounding areas?
    The chronic illness project is looking for health professionals including GPs, pharmacists and pharmacy assistants, allied health professionals and community health workers to assist researchers in a study conducted by the Griffith Health Institute.

    This study aims to fully explore the perspective of health consumers about the burden of chronic illness and multiple treatment regimes, their desire in terms of patient centred care and the related role of community pharmacy. Participation will form the basis of recommendations for models of care for community pharmacy to assist people with chronic disease and complex treatment regimes to better manage their illness.
