    Developed in 2013, the “Engaging Stakeholders in Identifying Priority Evidence-Practice Gaps and Strategies for Improvement in PHC” (ESP) project brought together the concept of knowledge co-creation and evidence on how to achieve large-scale change. It aimed to engage a wide range of stakeholders in using aggregated continuous quality improvement (CQI) data to identify priority gaps in care, barriers or enablers and strategies for improvement.


     


    The key areas of health care addressed with this phased approach are:



    1. Child Health

    2. Chronic Illness (vascular and metabolic)

    3. Rheumatic Heart Disease

    4. Mental Health and Wellbeing 

    5. Preventive Health

    6. Maternal Health
